Lots of faults happened to lead into Shinano's shameful sinking. Shinano was never fully fitted for damage / flooding control. Her CO was stupidly arrogant which is surprising since it was late 1944 with Japan losing badly and Allied Submarines were freely operating in strength in Japanese waters, which the IJN knew full well. What prompted Shinano's move was that the harbor she was being built at had undergone heavy air attacks. So the higher ups felt it was worth the risk of having her sail to another port than sit at the same one to get bombed to hell and back. It was a case of "Damned if you do, damned if you don't."

Kind of a risky gamble... Which turn pretty bad for them anyway.

Escorting Shinano to Kure (November 28-29, 1944)
Yukikaze: "I knew it... There was no chance of winning this war from the very beginning..."
Shinano, a Yamato class 3rd ship under construction, was to be moved to Kure under the escort of 3 destroyers so that she wouldn't be found again by the enemy B-29 reconnaissance plane. In the end she took 4 torpedo shots by one American submarine and sunk, resulting in history's shortest naval battle of just 17 hours. The crew members who were blamed for this incident could not hold their anger for "the constructors, who created a faulty ship, the HQ who shortened the secret trials and let her out of the dock, and the higher-ups who made the escort consist of only 3 destroyers".
