Well Lily, just like regular Saber, never grew up because of Caliburn and all that stuff so this is technically "Old" Lily.

I'm a little fuzzy on the details, but I've read that Merlin fixed Artoria up with male parts so she could have an heir. I've also read that Mordred is simply a homunculus clone of her.

on her 21st birthday, merlin turned artoria into a hermaphrodite as a practical joke, but she was bewitched by Morgan and had some sperm stolen before he could turn her back. Morgan then combined the sperm with one of her eggs and crafted Mordred.
