It'd be too confusing: Touhou has the likes of Flandre or Nue at extra stage, and Minoriko or Letty at 1st.
To put it bluntly: bigger boobs equals higher difficuly. Nothing to do with actual level placement. Your Flandre or Nue has "easy"(flat) boobs; Minoriko or Letty, as artists often portray, has "Hard" or "Lunatic"(C or D cups, I'm not good with american measurements) boobs. Yukari, Yuyuko and Yuugi are officially cosidered having "Phantasms". Hope you get the idea.

They pretty much are, albeit generally by stage order. Stage 5 bosses have a track record of being pretty big.

It's actually not that that consistent.

In what world are Yoshika, Doremy, and Shou NOT tittymonsters? Reisen and Sanae are pretty much always large to huge in my experience as well.
