Panties around one leg is a common thing in sex or toilet use pics. As for the panties being around the same leg twice I can only assume it is to prevent them from dangling onto the floor while she takes care of her business.
Edit: On a side note, do they still use toilets like these in Japan? Wouldn't a girl feel humiliated having to squat over a little hole in the floor?
