It's more about whether there is a physical difference between standard/kai and kai ni.
Basic Kiso seems to be fairly childish in form, while kai ni looks more adult, and there does seem to be actual breasts compared to a seemingly pretty flat standard form. You could read a cup size or two into the remodeling.
