Nudity is generally sexual. The part about "only based upon sexuality" is in reference to making things rated "safe" if there is blood and violence in an image.
One can have "safe" nudity (in the "Artistic Nudes"), and one can have "questionable" content fully clothed (overt pantyshots with underwear stretched to make labial folds extremely prominent and visible so that there's no question that it's the focus of the image to sexualize the pantyshot), but generally, nudity = questionable.
