She is some kind of secret weapon, like the title says: "ultimate weapon", that is used in military. And yes, she is partly human and partly machine, aka cyborg. Her body has problems with that machine part, it just can keep up through certain medicine. I guess the scar/wound is the result of this physical rejection.
But, it looks like she fell. Why else would there be all those cracks in the ground? Especially that little crater by her left shoulder; it looks like she broke her arm.

Could be that she fell, but so what. Doesn't change the fact, that the scar is from the human-mecha-conflict and that her clothes are torn, because of the transformation her body goes through when the machine part "shows up".
And I'm not sure about that "broken arm" idea. I don't think a bone in her body (if there are bones) could actually break; she has the whole molecular structure of her body under control when she is in that "attack mode" (as far as i know). Because of that a fall, even if a crater results of this, would not damage her that much.
