This might be a reference to MUGEN, the fighting game engine. People make and convert characters from fighting games, usually 2D, for use in MUGEN.
There is actually a MUGEN tag on Pixiv, if I remember correctly. It often features random mixes of characters from fighting games and Touhou, just like this.

I'll try to explain this picture as clean as I could.
In a video website called niconico douga, there is a genre called "MUGEN Story", where the uploader make up the storyline and alter the characters' background if needed. Mostly it involves some communications between various characters and MUGEN fights according to the story.
There is a MUGEN story called "Ryu ga saku!", in which Ryu is Sakuya's older brother. I think this pic is the reference to that.
