It is debatable since there are tellings (stories) of virgin births (miracle births) among gods and goddesses. However, if it isn't a miracle birth than yes, Suwako would be the only nonvirgin and has given birth.

That and I haven't seen anything to support that she actually has a childlike appearance. Perhaps in a certain ending to the game or some additional material might say that she is but I haven't seen it.
