The joke is far easier than you think and I am suprised I even have to explain it. In panel 3, Klara says that when you have a foreign lover nothing will stop you from learning a foreign language, to which Nonna replies in panel 4 with "If that is the case, then I can only go to Japanese universities" (directed to Katyusha) By this she implies that she loves Katyusha. That's the joke.
