The muscles aren't the issue. Look at the crotch. That's either the most swollen labia ever, or it's male genitalia.

The pixiv page has this titled "Shota Ganon-sama: 'Slave who is King in Name Only'". And the tags include such things as "shota" and "a ballsack you just wanna poke at".
