Uhh..this is normal for Fuutou, so it's not really against the TOS.

Just because it's normal for the character, doesn't mean it's exempt from the ToS. Though the size of her head in proportion to the rest of her personally bothers me more than anything else.

Wrong again. There are plenty of pictures from each category that fit in the huge category, at most.

The head is smaller than the breasts, WAY smaller.

Not exactly. Even pics with gigantic breasts might be approved, provided it doesn't make them a very important part of the pic. Clothed pics of women with such figures are more likely to pass than nudes or semi-nudes, for example. (This pic likely wouldn't have made it even without the huge tits, though, because of the other odd proportions and the sketchy lineart.)

Furthermore, just because a character's breasts are canonically one size, that doesn't mean that all depictions of them will be of that size. There are plenty of pics of Disgaea's Etna with big boobs, for example. Blanket quashing a character is not done for that reason.
