As I recall, it started when her IaMP portrait shows a fairly busty Sakuya (TH 7.5, as it was drawn by somebody else other than ZUN)... hence it was jokingly said that she wore pads.
I can't say when did it started (but it's mostly fandom), but wouldn't you be if you got a cute master like Remi? Not saying you got to be one, but it kind of tempts you.

Pad chief came about due to the difference between the art in the danmaku games (Zun art) and fighting games (Alphes art) Sakuya was always meant to have boobs but as we know Zun is not the best artist so the meme caught on that Sakuya pads her bra to explain the difference. Sakuya being a lolicon is taking what various characters call her "dog of the scarlet devil" and running wild with it.An interpretation of it that Sakuya loves Remilia so much thats she willing to do any order for Remilia no matter what it is. That saying also resulted in the Inu Sakuya meme.
